29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about April 26, 2022, at the Ensign Rig # 108; employees working in the Mud Logger Work Trailer were exposed to fire and electrical shock hazards in the following instances:  1) An orange extension cord being used by employees was connected to a relocatable power tap in series(daisy chained) to one another.  2) The relocatable power tap was permanently affixed to the wall of the trailer.
