29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: a) 3503 Willard St, Parkersburg: On or about May 2, 2022, two employees were exposed to a fall of approximately 12 feet eave to ground while replacing patching roof after removal of chimney. The employees were not utilizing a fall protection system or fall arrest harness.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Right side of home: On or about May 2, 2022 and times prior, employees were exposed to falls of 12 feet while using a Werner extension ladder to gain access to the roof to patch the roof after removal of chimney. The ladder was approximately 6 inches above the edge of the pick board that was installed on the roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): a) Right Side of House: On or about May 2, 2022, employees used a 25 foot Werner extension ladder to gain access to the roof. The ladder was placed at an angle that was approximately one half of the working length.
