29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  (a): Facility- Molding Department- A portable fire extinguisher was not readily accessible. Materials were placed in front of the fire extinguisher, obscuring its access, which could contribute to exacerbation of a fire, on or about May 3, 2022.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.157(e)(1): The employer shall be responsible for the inspection, maintenance and testing of all portable fire extinguishers in the workplace:  (a): Facility- Molding Department- A fire extinguisher was not properly inspected/maintained. An inspection tag located on the fire extinguisher indicated that the fire extinguisher was past due for inspection/servicing. The tag indicated that the last inspection/service was conducted in August of 2020 and was void in August of 2021. The fire extinguisher was observed, on or about May 3, 2022.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a): Facility- Injection Molding Robots - One or more methods of machine guarding was not provided to protect the employees in the machine area from hazards created by the moving robotic components. Employees are exposed to struck-by hazards from overhead when working on or near one of the eight injection molding machine robots, on or about May 3, 2022.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).       (b): Facility -Conveyor Strippers -  One or more methods of machine guarding was not provided to protect the machine operators from hazards created by the rotating conveyors. Employees are exposed to crushing and finger amputation hazards when using the conveyor strippers, on or about May 3, 2022.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Access to the circuit breaker panels was obstructed by material that was placed directly in front of the panels, on or about, May 3, 2022.
