29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Emblem Riverside Apartments located at 1551 Riverside Parkway, Douglasville, GA 30135, Garage #5, Roof: On or about 05/03/2022, the employer exposed employees to falls of 11-feet, 1-inch to the lower level, in that personal fall protection systems were not attached or worn incorrectly to prevent falls from a 6/12 pitched roof while engaged in plywood sheathing installation activities.

29 CFR  1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.   a) Emblem Riverside Apartments located at 1551 Riverside Parkway, Douglasville, GA 30135, Garage #5, Roof: On or about 05/03/2022, the employer exposed employees to falls of 11-feet, 1-inch to the lower level, in that personal fall protection systems was not provided with an anchor point as the life-line snap hook was wrapped around a roof truss and attached to itself and not connected to the anchor provided with the system or a comparable anchor system.

29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) Emblem Riverside Apartments located at 1551 Riverside Parkway, Douglasville, GA 30135, Rear side of Garage #5:  On or about 05/03/2022, the employer failed to inspect all electrical extension cords used to provide power to the Makita 5007F 7-1/4-inch Circular Saw, in that a red with blue stripe 100-ft. electrical cord was damaged at the strain relief on the male end exposing the internal energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
