29 CFR  1926.20(b)(1):  The employer did not initiate and maintain such programs as may be necessary to comply with this part:  On or about May 4, 2022 the employer did not have a program in place to identify hazards found on the job site of 13031 Wetzel Road in Van Wert, Ohio 45891. This program would assist in maintaining a safe working environment by supporting the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls from a maximum of 12.25 feet during job operations.

29 CFR  1926.503(a)(1):The employer failed to provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about May 4, 2022, the employer failed to provide training to employees on the adequate use of fall protection working at the site 13031 Wetzel Road in Van Wert, Ohio 45891. Employees were exposed to falls of approximately 12.25 feet from the top of the roof to the ground below while securing sheet roofing on the first story of a residential roof.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, personal fall arrest system, or provided with an alternative fall protection measure under another provision of paragraph 1926.501.  On or about May 4, 2022,  the employer, working at the address of 13031 Wetzel Road in Van Wert, Ohio, did not ensure employees were protected from fall hazards while nailing roofing material to the roof of a residential home. Employees working in this manner were exposed to falls of approximately 12.25 feet from the top of the roof to the ground below.
