29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: a) On May 4, 2022 and at times prior, fall protection was not provided for each employee performing work on a flat roof. Employees were exposed to a 30-foot fall hazard to the concrete below. Note) Certification of corrective action is required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) On May 4, 2022 and at times prior employees performing roofing work on a flat roof did not use the fall protection components (anchor points) in accordance with the manufacture's directions. Single anchor points installed on the roof were used by multiple employees at the same time. The manufacture's instructions clearly state single use only. Note) Certification of corrective action is required for this item.
