Farm Haus Luxury apartments, San Antonio, TX 78249: On or about May 5, 2022, and at times prior thereto, employees performing painting on siding were exposed to fall hazards while using an 8 foot a frame ladders in a closed and a lean-to manner.   (B.)	Farm Haus Luxury apartments, San Antonio, TX 78249: On or about May 5, 2022, and at times prior thereto, employees performing painting on siding were exposed to fall hazards while using an 4 foot a frame ladders in a closed and a lean-to manner.

29 CFR  1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  (A.)	 Farm Haus Luxury apartments, San Antonio, TX 78249: On or about May 5, 2022, and at times prior thereto, employees were exposed to fall hazards, while using an 8 Foot A frame ladder that was damaged, when gaining access to building siding.  (B.)	 Farm Haus Luxury apartments, San Antonio, TX 78249: On or about May 5, 2022, and at times prior thereto, employees were exposed to fall hazards, while using an 4 Foot A frame ladder that was broken, when gaining access to door frame.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS(S) AND/OR CONDITIONS)Note:  29 CFR 1926.21(b)(2) may be cited as an additional violation to support this requirement.  Farm Haus Luxury apartments, San Antonio, TX 78249: On or about May 5, 2022, and at times prior thereto, employees performing painting on siding did not recognize the fall hazards associated with using an A type frame ladder in a lean-to manner against the side of a wall.
