29 CFR  1926.651(c)(2): A stairway, ladder, ramp, or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in-depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.   On or about May 5, 2022, on the north side of the site, an employee was performing drainage repair in an excavation without a means of safe egress from an excavation four (4) feet or more in-depth.

29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  On or about May 5, 2022, the employees on the north side of the site were repairing a drainage line in an excavation where the spoils were not two feet from the edge of the excavation, and the mini excavator was sitting on the edge of the excavation.  Grady Crawford Construction Co, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard  1926.651(j)(2) which was contained in:  A) OSHA Inspection #1573232, citation number 1, item number 2, with a citation issued  April 11, 2022, and was affirmed as a final order on April 29, 2022, concerning a workplace located at Essen Lane and Suma Blvd/5063 Essen Lane, BATON ROUGE, LA 70814.

On or about May 5, 2022, on the northeast side of the site, the employee was repairing a drainage line in an excavation that was five feet or more in-depth with inadequate means of protection.
