29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Jose Hernandes located at 239 Schrock Road, Westerville, OH 43081:  On an preceding May 4, 2022, the employer did not ensure that an extension ladder used for access to the roof of a single story residential house had the side rails extending at least 3 feet beyond the upper landing nor was the ladders secured the top with a grasping device provided. The following condition exposed employees to falls up to 9 feet.
