29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  (a) Location - 253 Low Street,  Newburyport, MA  On or about 05/06/2022, employees were exposed to fall hazards while working in a bucket truck greater than ten feet above ground level with no personal fall protection.

29 CFR  1926.454(a):The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: [1926.454(a)]   (a)  Location - 253 Low Street, Newburyport, MA  On or about 05/06/22, employees were working in a bucket truck lift, neither employees had been trained in the use of fall protection.
