29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.  On or about May 09, 2022, during the replacement of a Sewer line at a worksite located at 9512 Vista Dr, North Royalton, OH 44133; the employer did not ensure the use of an appropriate ladder or other safe means of egress while an employee was working to install a sewer line in a trench at depths up to 8.6 feet. The employee utilized the sloped section at the East end of the trench as ingress/egress from the work location.

On or about May 09, 2022, during the replacement of a Sewer line at a worksite located at 9512 Vista Dr, North Royalton, OH 44133; Employees utilized a separated section of an extension ladder to exit a trench. The employer did not ensure that employees utilized an aluminum extension ladder in a manner for which it was designed.
