On or about May 11, 2022, at the address of 7725 Race Rd. North Ridgeville, OH 44035: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury/illness or death.

29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a)	Expert Commercial Roofing LLC. on or about May 11, 2022, at the address of 7725 Race Rd. North Ridgeville, OH 44035: The employer did not ensure that 8 dome skylights measuring 4' X 7' on a low slope roof was guarded properly to prevent employees from falling through approximately 18 feet to the floor below. One of six employees performing roofing work activities fell through one of the skylights 18feet - 3inches to his death onto the concrete floor below.

North Ridgeville, OH 44035: The employer did not ensure employees were protected from falls while a top of a low sloped roof. Employees removing and replacing fabric reinforced roof system atop the structure were exposed to falls up to 18ft to the ground below.

North Ridgeville, OH 44035: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.
