29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) Magnolia Pointe Subdivision located at Land Lot #1247 Dallas Acworth Highway, Dallas, GA 30132, Excavation: On or about 05/11/2022, the employer exposed employees to cave-in/engulfment hazards while installing 18-inch x 8-foot concrete sewer pipe, in that a ladder or other safe means of egress from the excavation was not provided to employees working in an excavation measuring 13-feet wide by 7-feet, 6-inches deep by 25-feet long.
