29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Fort Indiantown Gap, Building 7-74 Police Station - An employee was working in an unprotected trench, approximately six feet below grade, while placing a pump into the trench to allow for the removal of accumulated water, on or about May 11, 2022. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) Fort Indiantown Gap Building 7-74 Police Station - Employees were working in a trench utilizing a 6 foot Werner Step ladder and a 6 foot Werner Lean Safe Step Ladder in the closed position to enter and exit the trench, on or about May 11, 2022. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
