29 CFR 1904.7(a):  The employer did not record workplace injuries and illnesses that would be considered to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness. You must also consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness:   a)  In the Facility, on or about May 12, 2022 - The employer did not record work-related COVID-19 illnesses which resulted in days away from work on the 2022 OSHA Injury and Illness Form 300 or equivalent.

29 CFR  1910.502(q)(2)(ii)(A):  The establishment's COVID-19 log did not contain, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced:  a)  In the Facility, on or about May 12, 2022 - The employer failed to ensure the COVID-19 log contained one form of contact information, occupation, location where the employee worked and the date of the employee's last day at the workplace for each entry on the Positive COVID-19 Staff List.
