On or about May 10, 2022, at the above addressed jobsite, an employee performing residential roofing operations was exposed to fall hazards due to the anchorage point for the employee's personal fall arrest system not being capable of supporting at least 5,000 pounds.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about May 10, 2022, at the above addressed jobsite, an employee performing residential roofing operations, approximately fifteen (15) feet six (6) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  Enrique's Remodeling Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1506091, citation number 1, item number 1, and was affirmed as a final order on February 12, 2021, with respect to a workplace located at 2041 Wren Rd. Lot 27 in Yorkville, Illinois.

On or about May 10, 2022, at the above addressed jobsite, the portable extension ladder used to access and egress the roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to hazards associated with falls.  Enrique's Remodeling Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1506091, citation number 1, item number 2, and was affirmed as a final order on February 12, 2021, with respect to a workplace located at 2041 Wren Rd.
