29 CFR  1926.451(b)(1):  Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide:  a.   At the residential construction job site, on the second level of the scaffold system, at 839 Terrapin Drive in Debary Florida.  On or about May 6, 2022, and at times prior, three stucco masonry employees were exposed to a 13.4-foot fall hazard in that two scaffold planks were missing, creating a 1.6-foot wide gap at each end of the scaffold deck that the employees could fall between.

29 CFR  1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a.   At the residential construction job site, on the second level of the scaffold system, at 839 Terrapin Drive in Debary Florida.  On or about May 6, 2022, and at times prior, three stucco masonry employees were exposed to a 13.4-foot fall hazard in that end rails were not installed at each end of the decking.

29 CFR  1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   At the residential construction job site, on the front porch roof, at 839 Terrapin Drive in Debary Florida.  On or about May 6, 2022, a stucco masonry employee was exposed to an 11-foot fall hazard from the eave of the 4:12 pitch roof in that he wasn't protected by a fall protection system.

At the residential construction job site, on the front porch roof, at 839 Terrapin Drive in Debary Florida.  On or about May 6, 2022, a stucco masonry employee was exposed to an 21-foot fall hazard from the extension ladder that was placed on the 4:12 pitch roof without tie-offs, fasteners, or cleats.
