29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system:  On or about May 12, 2022, employees conducted roofing activities on a low-sloped roof above 6 feet from ground level without a fall protection system in place.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about May 12,2022, at this worksite, employee engaged in roofing activities, utilized a ladder to access/egress the roof that did not extend at least three feet above the roof landing.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  On or about May 12, 2022, at this site, the flexible cord powering an air compressor was not connected with a functional strain relief device.
