On or about March 12, 2022, and at times prior thereto, the exit route doorway for the back stockroom was blocked and obstructed while employees were working within, exposing the employees to various fire hazards.

A continuing and effective extermination program was not instituted where their presence is detected.  On or about March 12, 2022, and at times prior thereto, a continuing and effective extermination program was not instituted where the presence of rodents were detected, exposing employees to various health hazards.  1) Deceased rodents within stockroom office; 2) Living rodents within stockroom walkways;  3) Rodent Feces within establishment offices to include stockroom, employee breakroom, and conference room.
