The Line 3 Stacker Laborer periodically removes jammed or fallen material from between conveyor sections without locking out the conveyor section(s). The Line 3 Stacker Laborer was not trained as an authorized employee.  b) On or about May 19, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI in Building 11, a Machine Operator cleaned equipment in Building 11, including the Straight Line Rip Saw and the Mereen-Johnson Co. Double End Tenoner, without locking out the  equipment before cleaning. The Building 11 Machine Operator was not trained as an authorized employee.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about May 17, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 5-6, a Line 3 Stacker Laborer cleaned equipment on Line 3, including under conveyor lines, without locking out the equipment being cleaned. The Line 3 Stacker Laborer periodically removed jammed or fallen material from between conveyor sections without locking out the conveyor section(s).  b) On or about May 19, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI in Building 11, a Machine Operator cleaned equipment in Building 11, including the Straight Line Rip Saw and the Mereen-Johnson Co. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc:  a) On or about May 17, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 11, the Finger Jointer Even End (Corner Spot) Work Station Laborer was exposed to ingoing nip points from the feed rolls meeting the top of the material being moved.  b) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 5-6, the Finger Jointer Even End (Corner Spot) Work Station Laborer was exposed to ingoing nip points from the feed rolls meeting the top of the material being moved.  c) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 5-6, the Finger Jointer Even End (Corner Spot) Work Station Laborer was exposed to pinch points from the gray material feed slat conveyor broken slats spaces traveling under the table surface.  d) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 7, the Line 4 Keim Multi-Rip Saw Operator is exposed to crush points where the Keim Multi-Rip In-Feed System feed rolls lower onto the material being conveyed into the Keim Multi-Rip Saw.   e) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 7, employees stacking from the Line 4 The Brute Chop Saw off bearing Conveyor #20 were  exposed to an ingoing nip point in the tail pulley area. The tail pulley ingoing nip point beside the nearby connected conveyor unit was not guarded.  f) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 7, employees unstacking from the Baker Band Resaw Model ABX, Serial Number 07-3949 stacking conveyor unit were exposed to ingoing nip points in the tail pulley area.  g) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI, in Building 5-6, employees unstacking from the Line 3, conveyor #158 were exposed to a chain and sprocket  tail pulley ingoing nip point.  h) On or about May 18, 2022, at the facility located at N1308 Drumlin Drive, Campbellsport, WI in Building 4, on Line 1, the Eagle Transfer work station employee was exposed to a  chain and sprocket tail pulley ingoing nip point on the second line from the work station.  i) On or about May 18, 2022, at a facility located at N1308 Drumlin Drive, Campbellsport, WI in Building 4, the Line 1 Sander Feeder was exposed to a chain and sprocket tail pulley ingoing nip point.  j) On or about May 18, 2022, at a facility located at N1308 Drumlin Drive, Campbellsport, WI in Building 10, Pre-Finish Area, the Makor Finishing Line Catching Station employee was exposed belt and pulley conveyor tail pulley ingoing nip points.

29 CFR  1910.213(i)(1): All portions of the saw blade shall be enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table. Bandsaw wheels shall be fully encased. The outside periphery of the enclosure shall be solid. The front and back of the band wheels shall be either enclosed by solid material or by wire mesh or perforated metal. Such mesh or perforated metal shall be not less than 0.037 inch (U.S. Gage No. 20), and the openings shall be not greater than three-eighths inch. Solid material used for this purpose shall be of an equivalent strength and firmness. The guard for the portion of the blade between the sliding guide and the upper-saw-wheel guard shall protect the saw blade at the front and outer side. This portion of the guard shall be self-adjusting to raise and lower with the guide. The foot petal activation lowers the saw blade hood onto the material being cut, like a hold down, and then the saw blade raises and cuts the material.  b) On or about May 18, 2022, at the facility located N1308 Drumlin Drive, Campbellsport, WI, in Building 5-6, Finger Joint Line, the Operator's hands of the Precision Products Company Model 14M Up Cut saw, Serial Number 14332 were not prevented from reaching under the saw blade guard before and during depressing the foot pedal control. A bolt had been installed to prevent the saw blade guard from lowering before the saw blade rises to cut the material.  c) On or about May 18, 2022, at the facility located N1308 Drumlin Drive, Campbellsport, WI, in Building 3, near the Line 5 Turn Table,  the Operator's hands of the Precision Products Company Model 14 Up Cut saw, Serial Number 14-1090 were not prevented from reaching under the saw blade guard before and during depressing the foot pedal control. The foot petal activation lowers the saw blade hood onto the material being cut, like a hold down, and then the saw blade raises and cuts the material.
