29 CFR  1910.243(c)(3):Vertical portable grinders. Safety guards used on machines known as right angle head or vertical portable grinders shall have a maximum exposure angle of 180 deg., and the guard shall be so located so as to be between the operator and the wheel during use. Adjustment of guard shall be such that pieces of an accidentally broken wheel will be deflected away from the operator.  On or about May 24, 2022, employees were exposed to struck-by hazards while operating hand held grinders without the use of  safety guards in place.

29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  On or about May 24, 2022, the employer failed during calendar year 2021, to electronically submit information from their OSHA Form 300A or equivalent by 05/24/2022.  a). The establishment operates under two different NAICS codes: 1: Employed 26 employees and is classified 336214 - Travel Trailer and Camper Manufacturing  during calendar year 2021. 2: Employed 17 employees and is classified 332999 - All Other Miscellaneous Fabricated Metal Product Manufacturing during calendar year 2021.
