For falling objects too large, heavy, or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. On or about December 3, 2021, employees on the site's East side were performing masonry work on a scaffold with no toe-boards. Employees were not wearing head protection and were not protected from falling objects.
