Employees on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location - 2 Mudge Way, Bedford, MA On or about 05/19/22, employees were exposed to a fall hazards greater than 10 feet while placing concrete on second story steel decking.
