29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about 5/19/2022 an employee at the 185 Main Street, Niantic framing project was exposed to approximately 18' falls without fall protection while engaged in framing activities from an aerial lift.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 5/19/2022 employees at the 185 Main Street framing project were exposed to approximately 18' falls due to missing top rails while engaged in framing activities.
