29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Building Exterior-left side of the building:  Employee were exposed to eye/face injuries while laying block without the use of eye and face protection. Employees were not protected with any type of eye/face protection wile using mortar cement, on or about 5/18/22

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) Building Exterior left side: Employees were using pipe scaffold frames and cross braces to access the scaffold, exposing employees to a fall of approximately 15 feet to the ground surface, on or about 5/18/22
