29 CFR  1910.23(b)(8):  The employer did not ensure that ladders are used only for the purposes for which they were designed.   630 Main St., Springfield, MA: On or about  May 24, 2022, an employee was observed working from an A frame step ladder with one foot on a step and one foot on a back side support ( non step) of the ladder, and was also observed leaning outside the side rails to perform her work function and the ladders use instructions advise against using the ladder in this manner.

29 CFR  1910.30(b)(1):  The employer did not properly train each employee  on the proper care, inspection, storage, and use of equipment covered by this subpart before an employee used the equipment:   630 Main St., Springfield, MA: On or about  May 24, 2022, an employee was observed working from an A frame step ladder while standing on both sides of the ladder and leaning outside the side rails to perform her work function and the exposed  employee had not  been trained on the ladder manufacturers use instructions which advises against these uses of the ladder.
