29 CFR  1910.178(m)(6):The employer did not ensure that safe distance was maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car:  a) Facility, 700 North 3rd Avenue, Altoona, PA:  On or about May 25, 2022, 3 employees operate forklifts in the back of the warehouse to pick items for customers on a shelving unit that is approximately 14' away from a rail car platform.  By the time they back up enough to turn, they are about a foot away from the edge of the 4'.5" deep rail pit platform that is not equipped with a curb or bumper.
