29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): (a): Exterior pipe scaffold: Employees working on the first and second level of the scaffold were exposed to fall hazards of approximately 6 to 13 feet onto the ground below below due to scaffold deck not being fully planked, on or about 5/20/22.
