29 CFR  1926.501(b)(4):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about May 9, 2022, and times prior thereto, employees, performing roof work, were exposed to fall hazards through skylights while not utilizing any form of fall protection.

29 CFR  1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.   On May 09, 2022, employees were exposed to fall hazard while conducting roofing work on a warehouse.  Employees gained access to the roof with a ladder mounted on the back of a company truck bed.
