29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) 1419 North Cameron Street Harrisburg, PA, Building A6 - On or about May 18, 2022, plastic wrap, extension cords, and other accumulated debris was not kept cleared from work areas such as on the 30 foot tall main roof at or near the transition point of the 32 foot extension ladder. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) 1419 North Cameron Street Harrisburg, PA, Building A6 - On or about May 18, 2022, employees were tearing off and installing a residential roof with a slope greater than 4 in 12, approximately 30 feet above the lower level above grade without fall protection. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) 1419 North Cameron Street Harrisburg, PA, Building A6 - On or about May 18, 2022, the portable extension ladder did not extend 3 feet above the landing surface at the main roof at the back of the residential building where the sub-roof which housed the HVAC was. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
