29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.    (a) Location - 441 Main Street, Rowley, MA:  On or about 5/26/2022, Symes Associates, Inc., as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person.  As a result, subcontractor employees were exposed to fall hazards in the following instances.  Instance a: Employees of Maria C. Construction Inc. were exposed to fall hazards of 30 feet or greater while installing shingles on a new roof.  Instance b: Employees of L. Garcia Corporation were exposed to fall hazards of 8 feet or greater while installing floor joist over an open and unprotected basement.
