29 CFR  1910.134(f)(2):Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used nor were employee(s) annually fit tested:  (a) Forest Street LTC, LLLP, 3345 Forest St., Denver, CO 80207: On May 31, 2022 the employer did not ensure that all employees who were required to wear a tight-fitting respirator while conducting normal job operations when an employee enters an isolation room were fit tested to the specific size, style model or make that was available in the employers inventory. Additionally, on May 31, 2022 the employer did not ensure that employees who were required to wear a tight-fitting respirator while conducting normal job operations when an employee enters an isolation  room were annually fit tested.
