29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. At a worksite located at 406 Yorkshire Drive, Saint Augustine, Florida, 32092: On or about May 20, 2022, the employer exposed employees to struck by hazards, in that protective helmets (hard hats) were not utilized by employees working at ground level while passing up sheets of plywood to another employee at the roof.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On a 7:12 pitch roof at a residence,  located at 406 Yorkshire Drive, in Saint Augustine, Florida, 32092, on or about May 20, 2022,  the employer exposed employees to fall hazards, in that an employee was doing sheathing work without using a fall protection system.  b. On the ceiling joist at a residence, located at 406 Yorkshire Drive, in Saint Augustine, Florida, 32092, on or about May 20, 2022, the employer exposed employees to a 10.6 foot fall hazard, in that there was an employee doing framing work on the joists without using a fall protection system.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a. At a new residence, located at 406 Yorkshire Drive in Saint Augustine FL, 32092: on or about May 20, 2022, the employer exposed employees to fall hazards in that, the employer allowed employees to use the top steps of a 6-foot portable A-Frame type ladder, to gain access and egress from the trusses, while performing framing operations.
