29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced.  a) Warehouse - On or about June 3, 2022, the wooden ten (10) foot ladder had deficient locking spreader bars and rung bracings.  Employees that used the ladder were, thereby, exposed to a fall hazard.

29 CFR 1910.23(e)(1)(viii): The employer did not ensure that no mobile ladder stand or platform moves when an employee is on it.  a) Warehouse - On or about June 3, 2022, the six (6) foot high mobile ladder platform had a defective locking mechanism that did not stop the ladder from moving when in use. Employees that used the mobile ladder platform during daily operations were, thereby, exposed to a fall hazard.
