29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Location: a) Throughout worksite; 4th floor to backyard a) On or about 07/13/2022, employees worked in proximity to the energized electric cord that was placed in a wet/damp location such as a puddle of water on the 4th floor of the worksite. The insulation jacket was frayed in multiple parts of the cord.
