This was most recently documented on or about May 25, 2022, and at times prior to, when the employer did not provide walking-working surfaces that were free of hazards such as accumulation of fry sawdust.  Dry sawdust, a combustible and explosive dust, was observed in the facility on elevated surfaces at a depth greater than 1.5 inches.

This occurred on or about August 17, 2022, and at times prior to that. The relocatable power tap was being used by employees to power tools and equipment.
