29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  WORKSHOP/BINDERY DEPARTMENT:  The rotating ingoing nip points on the rollers of the following equipment were unguarded.  1. The top roller of the  Pot Devin Rotary steel rubber roller press was unguarded.  2.  The rollers of the Pot Devin cold glue machine were unguarded.  3.  The rollers of the small Pot Devin cold glue machine were unguarded.

29 CFR  1910.305(g)(1)(iv)(D):Flexible cords and cables were attached to building surfaces:  WORKSHOP/BINDERY DEPARTMENT:    1)  The flexible cord attached to  the Pot Devin Rotary Steel Rubber Roller press was wrapped around the truss traveled over head to the bus bar.  2) The flexible cord attached to the Pot Devin small cold glue was held buggie cord, plugged into a surge protector, plugged into a larger plug/cord, then traveled to the building structure and attached to the junction box.
