29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) On or about June 3, 2022, an employee was servicing a Ford F350 dual pickup truck on a inclined surface and did not utilize an energy isolating device to ensure the vehicle could not roll down the inclined surface.  This exposed employees to a crushing and struck by hazard.
