29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) New townhome construction located on Lot #66 Spike Trail, Smyrna, GA 30082, Roof: On or about 06/07/2022, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees operating this equipment at 140 PSI engaged in framing and sheathing activities.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New townhome construction located on Lot #66 Spike Trail, Smyrna, GA 30082, Roof: On or about 06/07/2022, the employer exposed employees to fall hazards of approximately 32-feet, 2-inches from the lower ground level, in that a fall protection system was not provided to employees engaged in framing activities on a 6/12 pitched roof.

29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) New townhome construction located on Lot #66 Spike Trail, Smyrna, GA 30082, Front side of Unit #66: On or about 06/07/2022, the employer failed to inspect all electrical extension cords used to provide power to the Makita 5007F 7-1/4-inch Circular Saws plugged into the multi-way power cord which had a damaged strain relief exposing the internal insulated wires. An inspection of these cords were not performed prior to the shift as these cords were not repaired or removed from service.
