(Bentley Brook Building)., Hancock, MA : On or about June 7, 2022, several employees observed roofing this residential building were attached to anchors that were improperly installed with nails instead of the required screws  invalidating the manufacturers  5,000 pound load per employee rating. (Bentley Brook Building)., Hancock, MA : On or about June 7, 2022, several employees were observed climbing down the roof access ladder which was set  such that the horizontal distance from the top support to the foot of the ladder was approximately one-third of the working length of the ladder.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
