29 CFR 1926.651(c)(2): A stairway, ladder, ramp, or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in-depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about June 7, 2022, employees on the south side of the site were installing a drainage pipe in an excavation that was 4 feet or more in-depth that did not have a stairway, ladder, or other safe means of egress.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as but not limited to broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. On or about June 7, 2022, employees used a damaged ladder on the site's south side. The ladder had structural defects and a missing rung.
