29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed through a written certification that identifies the workplace had been evaluated; the person certifying that the evaluation had been performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment.   On or about June 08, 2022. the employer did not certify through a through a written certification that the workplace hazard assessment for personal protective equipment (PPE). This condition exposed employees to struck-by , caught-between hazards, sharps, and fall hazards.

29 CFR  1910.141(a)(4)(ii): All sweepings, solid or liquid wastes, refuse and garbage was not removed in such a manner as to avoid creating a menace to health and as often as necessary or appropriate to maintain the place of employment in a sanitary condition.   On or about June 08, 2022, employees were exposed to possible injuries and illness due to unsanitary conditions. Garbage, solids, or liquid wastes, not being removed in such a manner as to avoid creating a menace to health.

29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m).  On or about June 08, 2022, employees engaged in trash removal activities were exposed to fall hazards while working from heights above 4 feet without fall protection.
