29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) 2080 Terry Rd, right side of house: On or about June 8, 2022 and times prior, an employee used a fabricated frame scaffold to paint the side of a residential home. The employee worked from the 4th buck of the scaffold that was only equipped with one plank and exposed the employee to a fall of 24 feet. b) 2080 Terry Rd, right side of house: On or about June 8, 2022 and times prior, an employee used a fabricated frame scaffold to paint the side of a residential home. The employee worked from the 3rd buck of the scaffold that was only equipped with one plank and exposed the employee to a fall of 15 feet.

The employee was exposed to a potential scaffold collapse due to the scaffold poles resting directly on wood, concrete and cinder block instead of a base plates.

The scaffold was not erected by a competent person qualified in scaffold erection in that the scaffold was not fully planked, guard rails were not in place, and base plates did not rest on a solid/firm foundation.

There were no guardrails installed on the 4th buck of the scaffold and personal fall protection was not used exposing the employee to falls up to 24 feet. b)2080 Terry Rd Right Side Middle Tier: On or about June 8, 2022 and times prior, an employee used a fabricated frame scaffold to paint the side of a residential home. There were no guardrails installed on the 3rd buck of the scaffold and personal fall protection was not used exposing the employee to falls up to 15 feet.
