Employees service and maintain equipment including, but not limited to the Amanda and Durma press brakes, without documented lockout/tagout procedures. This hazard was last observed on or about August 24, 2022.

29 CFR  1910.1026(d)(1): The employer did not determine the 8 hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI) in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section.   The employer is failing to determine the 8 hour time-weighted average hexavalent chromium (chromium VI) exposure for each exposed employee using the monitoring or performance options. Hexavalent chromium exposure determinations were not available upon agency request for employee(s) engaged in stainless steel welding. This violation was last observed on or about August 24, 2022.

29 CFR  1910.1026(i)(5): The employer did not ensure that employees do not eat, drink, smoke, chew tobacco or gum, or apply cosmetics in regulated areas, or in areas where skin or eye contact with chromium (VI) occurred:   The employer is failing to ensure that employee(s) exposed to hexavalent chromium do not drink in regulated areas. An employee engaged in stainless steel TIG welding kept and drank water at his welding station. This hazard was last observed on or about August 24, 2022.
