29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Establishment- The employer failed to provide  adequate fall protection training for employees who are exposed to fall hazards of approximately 8 feet and 9 inches.

29 CFR  1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   805 Blueberry Lane, Waterboro, ME- The employer did not provide employees acceptable means of access or egress  to a working platform at a height of approximately 8 feet and 9 inches above the next lower level. The means of access and egress being used by employees were the stringers without the risers and treads, exposing employees to fall hazards.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  805 Blueberry Lane- Employees were exposed to falls greater than 8 feet to a lower level due to no fall protection.  DSD Construction Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1399723, citation 1, item 3 and was affirmed as a final order on February 28, 2020, with respect to a workplace located at 530 Main St, Melrose, MA.
