29 CFR  1910.178(p)(1):  If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition:  On or about June 7, 2022,  and at times prior to employees were exposed to struck by hazards from holes being drilled in the forks weakening the stability of the forkolift tines.

29 CFR  1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit."  On or about June 7, 2022, and at times prior to, at 825 West 68th Street, Odessa, Texas, metal fabrication shop the employer failed to have an outdoor exit clearly marked and visible: 1. Exit door East side of shop.
