29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about June 13, 2022, employees working on roofing tasks were exposed to fall hazards on the west side of the site by utilizing a site-built material box attached to an all-terrain forklift to exit the roof. The job-built box that did not have guardrails/fall protection along the open sides.
