29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:  a. On or about 6/14/22 adjacent to the south side of the building: One employee was positioned inside a raised RB4000 Equipter, exposing the employee to a fall hazard of at least 12 feet, in violation of the manufacturer's specifications and limitations.

29 CFR 1926.1053(B)(1): Where portable ladders were used for access to an upper landing surface, and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.) North side of the building, on or about 6/14/22: Three employees used a portable extension ladder for access to the roof, with side rails that were 29 inches above the landing surface, exposing the employees to a fall hazard of 27 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.) On or about 6/14/22, on the roof of the Homewood Suites, Liverpool NY:  Two employees were working on a sloped roof (7:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of up to 30 feet to the ground below.
