29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to the work environment to control or eliminate any hazards or other exposure to illness or injury.   a) At 4176 Split Rock Road, Camillus, NY., on or about 6/15/2022: The employer did not train workers to recognize and avoid fall hazards when working at greater than 6 feet. Workers performed re-roofing operations while exposed to fall hazards of approximately 12 feet to the ground.   b) At 4176 Split Rock Road, Camillus, NY., on or about 6/15/2022: The employer did not train workers to recognize and avoid fall hazards when utilizing ladders to access and egress roofs. Workers utilized ladders which were not extended three feet above the landing while exposed to fall hazards of approximately 12 feet to the ground while accessing and exiting the roof.
