29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  on or about June 16, 2022, on the north side of the site, employees were performing roofing work at heights greater than six feet from the lower level without fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about June 16, 2022, on the south side of the site, employees performing roofing work were in use of a Gorilla ladder that was not extended at least three (3) feet above the upper landing surface and not secured at any point.
